Parent Consent Form for Childcare
A parent consent form for childcare should be a list of separate decisions, not one signature. Photographs, local outings, sun cream, plasters, nappy changing - a parent may be happy with four of those and not the fifth.
For nurseries, after-school clubs, holiday camps and childminders.
Bundled consent is not consent
The single "I consent to the above" tick is the version that causes trouble. A parent who is fine with outings but not with photographs on social media has no way to say so, so they either tick it anyway or ring you - and either way you don't have a usable record.
This template splits every permission into its own box, including the distinction everyone forgets: photographs used inside the setting versus photographs published online. Those are genuinely different decisions and should be asked separately.
A record you can actually rely on
Consent that was given verbally, or on a paper form in a drawer, is consent you cannot produce when it matters. What you need is the wording they agreed to, the answers they gave, and the date - all in one place.

Questions
How do I keep track of which children can be photographed?
Export the responses and you have a column per permission. Most settings print that sheet and keep it wherever photographs are taken.
Do we need to re-ask each year?
It's good practice, and it catches the parents who have changed their mind. Sending the same link annually gives you a dated history rather than one form from 2021.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
Yes - they see a confirmation on screen, and you can turn on an email so a copy of each submission lands in your inbox for your records.
Is it appropriate for children's information?
Responses are stored in your account and are never shared or used for advertising. Keep it to what you need for care and safety, tell parents what you hold and how long you keep it, and check your local rules on children's data before you publish.
