Daycare Registration Form
A daycare registration form is the longest form a parent will fill in for you, so it should be the only one. Days and hours needed, the child's routine, medical details, permissions and contacts - in short steps rather than one wall.

Pattern of care is the question that decides whether you can offer a place
Availability is not a headcount - it's a grid. Two children needing Mondays and Tuesdays fit where four needing all week don't. Until you know the exact days and hours, you cannot say yes or no.
That's why the days-and-hours question comes early here, with the desired start date next to it. The rest - routines, weaning, nap times, toileting - matters enormously on the first morning but not to the offer, so it sits in a later step where it won't put anyone off finishing.
One list, not five threads
Registrations that arrive by email, text and word of mouth become a headcount nobody trusts. You find out on the day that two people thought they were signed up and one wasn't.

Emergency contacts and permissions, not a paper pile
Allergies, medication, who may collect a child, photo permissions - these are the details you must be able to find in seconds, and a folder of paper forms is the worst possible place to keep them.

Questions
Should registration and the waiting list be the same form?
They can be, with a question asking whether they want a place now or to join the list. Many settings prefer two forms so the waiting-list version is much shorter.
What about funded hours?
Add a question asking whether they'll claim funded hours and from which term - it's the detail most likely to be missed, and it changes your invoicing.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
Can I get the list as a spreadsheet?
Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.
Is it appropriate for children's information?
Responses are stored in your account and are never shared or used for advertising. Keep it to what you need for care and safety, tell parents what you hold and how long you keep it, and check your local rules on children's data before you publish.
