Daycare Emergency Contact Form

One contact is not enough, and everyone learns that the hard way

The first number is in a meeting. Always. What makes the difference at 3pm is a second contact who is not the same person's other phone - a grandparent, a partner, a neighbour with a key.

The other field that earns its place is authorised collection. A named list, in writing, dated, is what protects staff on the afternoon someone unexpected arrives at the door saying they were asked to come.

Questions

How often should this be refreshed?

Once a year at minimum, and whenever a family moves or changes number. Because responses are dated, you can see at a glance which records are getting old.

Can we print it for the register?

Export the responses and you have every child's contacts in one spreadsheet - which is what most settings print for the folder that lives by the door.

Can I stop registrations when we're full?

Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.

Can I collect one registration per person or per group?

Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.

Can I get the list as a spreadsheet?

Is it appropriate for children's information?

Responses are stored in your account and are never shared or used for advertising. Keep it to what you need for care and safety, tell parents what you hold and how long you keep it, and check your local rules on children's data before you publish.
