Camp Waiver Form
A camp waiver form covers the ground a camp actually needs covered: which activities a child may take part in, who to call, what medical treatment is authorised, and whether photographs are allowed.

Authorisation matters more than the signature

Is this legally binding?
Treat it as a record of permission and information, not as a substitute for legal advice. Have your insurer and, if you have one, your legal adviser check the wording before you rely on it - especially the treatment authorisation.
Can I collect one form per child?
Yes, and you should - each child needs their own medical details and permissions. Parents with several children fill it in more than once, which takes a minute each and is far safer than one shared form.

Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
Yes - they see a confirmation on screen, and you can turn on an email so a copy of each submission lands in your inbox for your records.
Is it appropriate for children's information?
Responses are stored in your account and are never shared or used for advertising. Keep it to what you need for care and safety, tell parents what you hold and how long you keep it, and check your local rules on children's data before you publish.
