Material Request Form
A material request form is an internal pull from stores. Part number, quantity, which work order it's for, and when it's needed on the line.

A work order reference makes the cost land in the right place
Untracked material issues are how a job's cost becomes fiction. Stores hands out parts, nobody records which job used them, and the estimate that looked profitable turns out not to have been.
So the work order or job number is required here, next to the quantity. It also asks whether this is a normal issue or a replacement for scrap - which is the only way anyone ever finds out how much material is being lost to rework.
Take orders while you're working
You can't answer the phone with your hands full, and the person calling won't wait. A form takes the order at 11pm on a Sunday and has it waiting when you open.
Specifications, tolerances and quantities in writing
An RFQ described on the phone is an RFQ you'll re-quote. Material, quantity, tolerance and delivery date belong in fields, where they can't be misheard and can be forwarded to the shop floor exactly as received.
The same discipline suits internal forms - maintenance requests, visitor registration, incident reports - where consistency is what makes a month of records readable.

Questions
Should we ask whether it's replacing scrap?
Yes, and don't make it feel punitive. It's the only reliable source of scrap data most shops have, and knowing which parts are being remade tells you where to look.
Can this integrate with our ERP?
Point a webhook at it and every request is posted as JSON when submitted, or pull them from the REST API. A CSV export works for periodic reconciliation.
Can I take payment on the form?
Not yet. Most people use this to capture the order and then invoice or take payment on collection - which also means you can confirm availability before charging anyone.
Can I limit what people can order?
Yes. Options are lists you control, so only what you actually sell appears - and you can close the form when you're at capacity.
Will I be told about new orders straight away?
Yes, if you turn on live notifications: each order is emailed to you as it arrives. Otherwise they collect in your dashboard.
Can we use this behind the scenes as well as with customers?
Yes, and many do: the same account can hold a public RFQ form and internal forms for maintenance or visitors. Only your team needs logins.
