Factory Visitor Registration Form

The register is what a fire marshal reads
In an evacuation, someone stands at a muster point with a list. If the visitor register is a paper book at a reception nobody could reach, or a spreadsheet on a locked PC, that list doesn't exist.
A form gives you a live list accessible from any phone, with each visitor's host and mobile number. It also collects the PPE and induction requirements in advance, so a visitor arrives with steel toecaps rather than being lent a pair at the gate.

Questions
Should visitors register in advance or on arrival?
In advance is better - the host gets to check PPE and inductions before someone travels. Keep the same form available at reception on a tablet for unplanned visitors.
Can we use this for contractors?
For short visits, yes. Contractors doing work usually need a permit to work and a RAMS review, which is a separate and longer form.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
Can I get the list as a spreadsheet?
