Production status is the priority field
Maintenance queues are not first-in-first-out - they're ordered by what's costing money. A stopped line is different from a machine running with a noise, and a request that doesn't distinguish them makes a supervisor phone around to find out.
So this template asks about downtime and safety before it asks anything technical, and makes both required. It also asks what changed recently - a new batch of material, a different operator, a setting adjusted - which is the question that solves a surprising share of faults before an engineer touches anything.

Questions
Should we use this for planned maintenance too?
You can, with the request type as the first question - but planned work is usually scheduled from an asset register rather than requested. This form's real value is breakdowns and defects.
Can urgent requests notify the maintenance team immediately?

Can reports be submitted anonymously?
