Training Registration Form

A training registration form should capture what someone wants to be able to do afterwards. Without that, training gets measured in attendance rather than in anything changing.

Objectives now, so evaluation is possible later

Most training evaluation fails because nothing was written down at the start. Six weeks later, nobody can say whether it worked, because nobody recorded what "working" would have looked like.

Asking each participant what they want to be able to do differently takes them a minute and gives you two things: a trainer who can adjust the session to the actual room, and a baseline to ask about afterwards. It also quietly filters the people who were sent and don't know why.

Questions

Should we require manager approval on the form?

Asking whether it's been discussed is usually enough - a required approval field turns a registration into a workflow. This template asks, and flags the ones that haven't.

Can we use this for mandatory compliance training?

Yes, though the objectives question matters less. What matters there is a dated record of who registered and attended, which the export gives you.

Can I stop registrations when we're full?

Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.

Can I collect one registration per person or per group?

Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.

Can I get the list as a spreadsheet?

Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.

Can employees submit anonymously?

Yes, if you make the name optional - useful for feedback and concerns. Nothing else is recorded about a submitter: no IP address, no browser fingerprint, no account.
