Time Off Request Form

A time off request form is the highest-volume form in any company, so it should be four fields and thirty seconds. Dates, type, cover, submit.

The cover question is what makes approval instant

A manager approving leave is really asking one thing: what breaks while you're away? If the request answers it, approval takes ten seconds. If it doesn't, it sits in an inbox for a week generating a conversation.

So this template asks who's covering and whether anything falls in that window - a deadline, a client visit, a rota. It's one extra field for the employee and it removes the entire back-and-forth.

Questions

Can this track remaining entitlement?

No - it collects requests, and your HR system or spreadsheet holds the balance. Asking the employee to state their remaining days keeps everyone honest without another system.

Should different leave types use different forms?

One form with a type dropdown works for most. Sickness is often better separate, because it's reported after the fact rather than requested in advance.

Can applicants attach a CV or document?

Not yet - file uploads are on the roadmap. Most people ask for a link (to a portfolio, a CV in cloud storage, or a profile) which works today and keeps the form fast.

Can I close applications on a deadline?

Yes. Close the form and anyone with the link sees a short closed page instead of the questions, so nobody fills in an application that can't be considered.

Can more than one person review them?

Yes - invite your team into the same account and they'll see the same responses. You can also export to a spreadsheet and score them wherever you like.

Can employees submit anonymously?

Yes, if you make the name optional - useful for feedback and concerns. Nothing else is recorded about a submitter: no IP address, no browser fingerprint, no account.
