Remote Work Request Form
A remote work request form should ask the applicant to solve the manager's problem for them: how the work gets covered, how you'll be reachable, and what happens if it doesn't work.

Remote requests get refused for reasons that are usually solvable - a rota, a handover, someone else's Tuesday. Managers decline because working it out is their problem and they have no time.
So this template asks the employee to propose the arrangement, including how their responsibilities are covered and how they'll be contactable. A request that arrives with the answer already in it gets approved far more often - and a request that can't answer it has told everyone something useful.

Applications that arrive as emails and attachments are impossible to compare - one has the dates, another has the budget, none of them have both. You end up rebuilding each one by hand before you can make a decision.

Questions
Should we ask about the home workspace?
Yes - most employers have a duty of care for a home workstation. Asking about the desk, chair and internet is both a safety question and a practical one.
Should requests be permanent or trialled?
A trial with a review date is easier to approve and easier to end. This template asks for one, which makes the whole conversation less binary.
Can applicants attach a CV or document?
Not yet - file uploads are on the roadmap. Most people ask for a link (to a portfolio, a CV in cloud storage, or a profile) which works today and keeps the form fast.
Can I close applications on a deadline?
Yes. Close the form and anyone with the link sees a short closed page instead of the questions, so nobody fills in an application that can't be considered.
Can more than one person review them?
Yes - invite your team into the same account and they'll see the same responses. You can also export to a spreadsheet and score them wherever you like.
Can employees submit anonymously?
Yes, if you make the name optional - useful for feedback and concerns. Nothing else is recorded about a submitter: no IP address, no browser fingerprint, no account.
