Ask for the constraint, not the wish
Technical problems are defined by their constraints: the database that can't be migrated, the team that doesn't know Go, the compliance rule that forbids a region, the deadline that can't move. A solution that ignores those is a wasted hour.
So this template asks what's fixed and what's negotiable, alongside the stack and the specific question. It also asks what they've already tried - which stops the consultation beginning with the three suggestions they rejected last month.

Questions
Should I ask for access to code or systems?
Not on the form. Ask what exists - a repo, a diagram, monitoring - and arrange access separately once there's an engagement. Never ask for credentials on a web form.
How technical should the questions be?
As technical as your clients are. A form that asks about database engines and traffic profiles signals competence to an engineer, and an account manager will forward it to one anyway.
Can I add or remove questions?
Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.
Where do the answers go?
Into your dashboard, where you can read, filter and export them as a spreadsheet. You can also have every response emailed to you the moment it arrives, or as a daily or weekly digest.
Do I need a website?

Can this feed into our issue tracker?
