Ask for a feedback commitment, and mean it
Most beta programmes fill up with people who wanted early access and never report anything. You end up with two hundred users, four bug reports, and no idea whether the thing works.
So this template asks explicitly whether they'll report issues and join a call, and it says plainly that the software will be rough. The people who tick those boxes are worth ten who just wanted the badge - and the ones who don't tick them have saved you both some time.
Make signing up the easy part
Enthusiasm has a short half-life. If signing up means finding an email address, writing a message and waiting, most of the people who meant to do it won't.

Questions
Should betas be open or invite-only?
Invite-only, selected from a form like this. An open beta gives you volume and no signal; a curated one gives you people who'll tell you what's broken.
What should I promise testers?
Only what you'll deliver - early access and a direct line to the team is usually enough. Promising free access forever is a decision you'll regret.
Can I close sign-ups when I have enough?
Yes - close the form and the link shows a short closed page instead of the questions. Reopen it whenever you like.
Can I email everyone who signed up?
Export the responses as a spreadsheet and use whatever you already send email with. ava42 collects the list; it isn't a mailing tool.
Do people need an account?
No. Only you have an account. Anyone with the link can fill the form in, with no sign-up and no app.
Can this feed into our issue tracker?
