A generic demo wastes everyone's forty minutes
A demo that walks through every feature is a product tour, and product tours don't sell. What sells is fifteen minutes on the two things this specific buyer said were painful.
So this template asks what they're trying to solve and which parts matter most, and asks who's attending. A demo to one operations manager and a demo to a manager, a developer and a finance lead are different presentations - and knowing in advance is the whole difference.
Requests you can actually schedule
A name and "sometime next week" isn't a booking. What you need is when they can come, what they want, and how to reach them if the slot moves.
This template asks all three, so the diary entry is a copy-and-paste rather than a conversation.
A bug report that doesn't need three follow-ups
"It's broken" costs a developer an hour of guessing. Steps to reproduce, expected versus actual, browser and version turn a report into something that can be triaged without a conversation.
Fields make people answer the questions they'd otherwise skip - and being able to filter a month of reports by area or severity tells you where to spend the next sprint.
Give them options, not a blank box
Open questions get vague answers. Offering the services and the times you actually have makes the request usable, and quietly steers people towards the slots you want to fill.

Questions
How long should a demo be?
Ask them. Offering 20, 30 and 45 minutes gets you an honest answer about how much attention they're prepared to give, which is itself qualifying information.
Should I ask about budget before a demo?
A range is reasonable for enterprise deals. For self-serve pricing, don't - the pricing page already did that job.
Does this book the slot automatically?
No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.
Can I limit the options people can pick?
Yes. The service and time-of-day lists are yours to edit, so people can only choose from what you actually offer. You can also close the form when you're full.
Will I know straight away?
Yes, if you want to be. Turn on live notifications and every request is emailed to you the moment it arrives; otherwise collect them and read them in your dashboard.
Can this feed into our issue tracker?
Point a webhook at your tracker and every submission is posted to it as JSON as it arrives, or pull responses from the REST API. There's also a CSV export for bulk import.
