HOA Amenity Reservation Form
An HOA amenity reservation form covers the clubhouse, the pool area and the party room - and its real job is establishing who is responsible if something gets broken.
For HOAs, residents' associations and community boards.
Name the responsible resident, in writing
Amenity bookings go wrong when a resident reserves the clubhouse for a party thrown by someone else. Damage happens, and the board discovers that nobody agreed who was accountable.
So this template asks for the resident's unit and requires them to confirm they will be present and responsible for the guests, the clean-up and the deposit. It's a small amount of friction that prevents the most common dispute a board handles.
Stop booking by phone tag
Half your residents would rather book at 10pm than call at 10am, and the ones who do call reach a voicemail while you're working. Every missed call is a booking that went somewhere open.
A request form takes bookings while you're busy, asleep or driving - with the details you need written down rather than remembered.
A paper trail the board can actually point to
Architectural requests, complaints and maintenance issues arrive by email, at meetings and in person - and then nobody can find what was approved, when, or by whom. That's how disputes start.
One form per process gives the board a dated list with identical fields, which is both faster to run and much easier to defend when a decision is challenged.
It's a request, not a calendar takeover
You stay in control: requests arrive, you confirm the ones that work. No syncing, no double-booking, nothing to install - just the details you need in the order you need them.

Questions
Can we take the deposit through the form?
Not directly - collect the reservation and take the deposit through your usual method. The form's value is the written acknowledgement of the deposit and the rules.
Can we prevent double bookings?
Reservations are requests that you confirm, so the board keeps control. Post the confirmed calendar somewhere residents can see it and most conflicts disappear.
Does this book the slot automatically?
No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.
Can I limit the options people can pick?
Yes. The service and time-of-day lists are yours to edit, so people can only choose from what you actually offer. You can also close the form when you're full.
Will I know straight away?
Yes, if you want to be. Turn on live notifications and every request is emailed to you the moment it arrives; otherwise collect them and read them in your dashboard.
Can several board members see the submissions?
Yes - invite them into the same account and everyone sees the same list. Members change often, so you can remove someone's access the day they step down.
