Freelance Client Intake Form

Ask who signs it off, and what you need from them
Freelance projects don't overrun because the work was hard. They overrun because feedback took three weeks, or the brand assets never arrived, or a fourth stakeholder appeared at the end with opinions.
So this template asks who the decision-maker is, how quickly they can give feedback, and what they'll need to provide - copy, images, access, logins. Asking on day one turns those from your problem into a shared plan, and it's the single best protection a freelancer has.
Why ask it all up front
Half-finished information is worse than none: it looks like you can begin, and then you can't. A client who fills in one page has told you enough to quote, schedule and prepare - without a single back-and-forth.
It also sets the tone. A form that asks precisely, and only once, reads as a business that has done this before. That impression is doing real work while your competitor is still emailing "quick question".
A brief is worth more than a call
You are the sales team, the delivery team and the invoicing department. The cheapest hour you can save is the one spent extracting a brief from someone who has never written one - so ask the questions in the order you need them answered.
A written brief is also your protection. Scope creep is much harder to argue when what was originally asked for is on the record, timestamped.

Questions
Is this too much to ask before a project starts?
Clients who care about the outcome are relieved by it. If someone won't answer eight questions about their own project, that's information too.
Should I send this before or after quoting?
Before. Half of these answers change your number, and the other half change your timeline. Quoting from an email and filling in the detail later is how freelancers end up working for less than they meant to.
Can I add or remove questions?
Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.
Where do the answers go?
