The effective date is the whole document
Mid-term changes create the gaps that cause refused claims. A customer moves house on the 3rd, tells the broker on the 12th, and assumes cover followed them - and if nobody recorded the intended effective date, the argument is unwinnable.
So this template makes the effective date required and asks whether the change has already happened. It also states plainly that changes aren't in force until confirmed, which is the sentence that prevents the most expensive misunderstanding in a broker's book.

Should changes take effect from submission?
No, and say so. Underwriting may need to accept the change or reprice it - a form that implies instant cover creates a liability you didn't agree to.
Can customers request several changes at once?
Yes, and this template allows it - but ask for the effective date per change if they differ. Bundled changes with one date are a common source of error.
Can reports be submitted anonymously?
Yes, if you make the name optional. Nothing about the submitter is recorded otherwise - no IP address, no account, no tracking.
Can this go to more than one person?
