Life Insurance Quote Form

A life insurance quote form asks personal questions, so it has to explain why it's asking them. Age, smoking, height and weight, and a short health history - each one changes the premium materially.

For brokers, agents and insurance teams.

Explain why you're asking, next to the question

People abandon life insurance forms at the health questions, not because they mind the answers but because it feels intrusive without context. Insurance forms are the one place where a sentence of explanation increases completion.

So this template says plainly why smoking status, height and weight and family history matter, and it says that answering honestly is what makes a policy pay out. It also asks about dependants and mortgage - because those are what the cover amount should actually be based on, and most people guess.

Fewer calls, better numbers

A phone call is a fine way to build trust and a terrible way to collect specifications - nobody remembers everything, and nothing is written down. Collect the facts in writing first, and use the call for the part that actually needs a human.

You'll also stop quoting jobs you didn't want: when the form asks about budget and timing, the mismatches disqualify themselves.

Enough detail to quote, not enough to be a nuisance

Quoting needs specifics - cover type, value, dates, claims history - and every extra question loses people. The answer is to ask the ones that change the premium and nothing else, in one short pass.

Claims and renewals work the same way: ask the facts that let you act, keep it short enough that a stressed client finishes it, and have it in writing from the start.

Price the right jobs

Every quote costs you time whether or not you win it. Asking about budget and timing up front isn't rude - it's how both sides avoid a week of conversation about a job that was never going to happen.

Make it yours in a minute

Change the wording, drop the questions you don't need, add the ones you do, put your own colour on it. Saving publishes it, and you get a link to paste into your website, your email signature or a message. Responses land in your dashboard and download as a spreadsheet whenever you want them.

Questions

How much cover do people usually need?

It depends on debts, dependants and income replacement - which is why this template asks about those rather than just asking for a number. Use them to sanity-check the amount requested.

Should we ask detailed medical questions here?

No. Ask the few that drive an indicative price - smoking, height and weight, major conditions - and leave full underwriting to the application. A quote form that reads like a medical gets abandoned.

Can I ask for photos or files?

Not yet - uploads are on the roadmap. In the meantime, most people ask for a link to photos, or reply to the response with an email asking for them. Everything else about the job is captured here.

Can I see where each quote request came from?

Yes. Add a hidden tracking parameter to the link (for example one link on your site and another on your Google profile) and it's stored with the response.

How fast can I put this live?

About a minute. Pick the template, change the wording, press save - saving is publishing - and you get a link to share.

Is it suitable for claims information?

For dates, descriptions and contact details, yes, and the timestamped record is useful. Don't collect full bank details or identity documents through a web form; ask for those through a channel designed for it.
