Meeting Request Form
A meeting request form exists to stop meetings without a purpose. What decision needs making, who needs to be there, and how long it should take.
For consultancies, advisers and professional firms.
A meeting with no stated outcome is a meeting that repeats
The reason recurring meetings breed is that the first one had no defined outcome, so it ended with "let's pick this up next week". Multiply that across an organisation and you get calendars with no space to work in.
So the first required field here is the outcome: what should be true when this meeting ends. It also asks whether the thing could be handled asynchronously, which is a question that quietly cancels a surprising number of meetings before they're booked.
Fewer no-shows, fewer surprises
No-shows and "I thought I booked the other thing" both come from the same place: a booking nobody wrote down properly. When the client types it themselves, there's a record, in their words, of exactly what was asked for.
A first impression that reads as organised
In professional services the enquiry process *is* a sample of your work. A form that asks precise questions and loads instantly says something that a generic "contact us" box cannot.
It also protects your time: the questions that qualify a piece of work - scope, timing, who decides - are far easier to ask in writing than in a first call.
Confirm in writing, immediately
The moment they submit, they see a confirmation and you get an email. No wondering whether it arrived - which is the thing that makes people phone you anyway.
One minute to make it your own
Every label, option and placeholder on this template is editable, and you can add whatever this template doesn't ask. Save to publish, share the link, and export the responses whenever you need them in a spreadsheet.

Questions
Isn't a form for booking a meeting excessive?
For an internal chat, yes. For anything involving several senior people or an external party, the two minutes spent stating a purpose saves an hour of everyone's time.
Should I ask who's needed versus who's optional?
Yes - it's the most useful distinction on the form. Most meeting bloat comes from inviting people who could have read the notes.
Does this book the slot automatically?
No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.
Can I limit the options people can pick?
Yes. The service and time-of-day lists are yours to edit, so people can only choose from what you actually offer. You can also close the form when you're full.
Will I know straight away?
Yes, if you want to be. Turn on live notifications and every request is emailed to you the moment it arrives; otherwise collect them and read them in your dashboard.
Can I route enquiries to the right team?
Ask what the enquiry is about in a dropdown and notify a shared inbox; the answer is in the email, so whoever triages can forward it once, correctly.
