Consultation Request Form

A consultation request form should make the first meeting useful. What they want to discuss, what they've already worked out, and what decision they're trying to reach.

For consultancies, advisers and professional firms.

Ask what decision they're trying to make

A consultation booked with no stated question becomes an hour of general conversation that everyone enjoys and nobody acts on. The client leaves with impressions rather than an answer, and doesn't come back.

So this template asks for the decision or question in one line, and asks what they've already considered. That lets you prepare properly - and it means the hour ends with something specific, which is what turns a consultation into an engagement.

Fewer no-shows, fewer surprises

No-shows and "I thought I booked the other thing" both come from the same place: a booking nobody wrote down properly. When the client types it themselves, there's a record, in their words, of exactly what was asked for.

A first impression that reads as organised

In professional services the enquiry process *is* a sample of your work. A form that asks precise questions and loads instantly says something that a generic "contact us" box cannot.

It also protects your time: the questions that qualify a piece of work - scope, timing, who decides - are far easier to ask in writing than in a first call.

Confirm in writing, immediately

The moment they submit, they see a confirmation and you get an email. No wondering whether it arrived - which is the thing that makes people phone you anyway.

Yours to change, immediately

Nothing here is fixed. Rewrite a question, reorder them by dragging, add your brand colour, delete half of it. Save and the live form updates - there's no separate publish step and no deploy to wait for.

Questions

Should consultations be free or paid?

Whichever you choose, state it plainly on the form with the duration. Paid consultations attract people who prepare; free ones attract volume. Ambiguity attracts complaints.

How much background should I ask for?

Enough to prepare, not a full brief. Two short questions - what's the situation, what have you tried - get you 80% of the value without becoming homework.

Does this book the slot automatically?

No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.

Can I limit the options people can pick?

Yes. The service and time-of-day lists are yours to edit, so people can only choose from what you actually offer. You can also close the form when you're full.

Will I know straight away?

Yes, if you want to be. Turn on live notifications and every request is emailed to you the moment it arrives; otherwise collect them and read them in your dashboard.

Can I route enquiries to the right team?

Ask what the enquiry is about in a dropdown and notify a shared inbox; the answer is in the email, so whoever triages can forward it once, correctly.
