Should I state a minimum spend on the form?

Yes, per room if they differ. It's the fastest qualifier available and it prevents the conversation everybody dreads at the end of a site visit.

How do I handle provisional bookings?

Say how long you'll hold a date and what confirms it. Ambiguity here is how venues end up double-holding a Saturday in December.

Does this book the slot automatically?
