Data Privacy Request Form

A data privacy request form handles the requests people have a legal right to make: a copy of their data, a correction, a deletion, or an objection. The clock starts the moment it arrives.

For security firms, compliance teams and risk managers.

The clock starts on receipt, so log it properly on day one

Data subject requests carry statutory deadlines - commonly one month, extendable in limited circumstances. A request that arrives as a sentence in a support email and gets noticed three weeks later has already consumed most of the window.

So this form exists to capture requests in one place, timestamped, with the type identified and the scope specified. It also tells the requester what identity verification will be needed and why - which is the step that most often delays a response.

The same facts, every time

When each report is written differently, patterns are invisible. Consistent fields mean you can look across a month and see the thing that keeps happening, rather than a folder of prose.

Consistent records, because consistency is the evidence

An incident log is only useful if every entry answers the same questions. Free-text emails give you a folder; identical fields give you something you can search, count and hand to an auditor.

Every submission is stored with the exact time it was made, which is usually the first thing anyone reviewing an incident asks for.

Easy enough to actually be used

A reporting process nobody follows produces no data. This one is a link that opens instantly on a phone and takes a couple of minutes, which is the only version that gets used on a busy day.

One minute to make it your own

Every label, option and placeholder on this template is editable, and you can add whatever this template doesn't ask. Save to publish, share the link, and export the responses whenever you need them in a spreadsheet.

Questions

Should identity documents be uploaded here?

No. Ask what evidence they hold and verify it through a secure channel. Collecting passport scans on a web form creates exactly the risk the request is about.

Can we charge for a request?

In most regimes the first request is free, with narrow exceptions for manifestly excessive or repeated requests. Check your own rules before mentioning a fee - and be careful, because charging incorrectly is itself a breach.

Can reports be submitted anonymously?

Yes, if you make the name optional. Nothing about the submitter is recorded otherwise - no IP address, no account, no tracking.

Can this go to more than one person?

Yes. Notification recipients are a list, so a report can reach a manager and a safety lead at the same time.

Can I export everything for an audit?

Yes - one click gives you a CSV of every response with its timestamp, which is usually exactly what an auditor or insurer asks for.

Can we prove when something was reported?

Every response carries the timestamp it was submitted, and exports include it. That is usually what's needed; ava42 is not a tamper-evident audit system, so for regulated evidence chains check what your framework requires.
